Freedom of Information

The South Huron Hospital Association (SHHA) falls under the Freedom of Information and Protection Act (FIPPA). This Act provides greater transparency and accountability for the information under the custody and control of SHHA as a public institution. Anyone has a right to make a request for access to a wide range of information held by the hospital.

Please note that FIPPA does not apply to personal health information (PHI). PHI is governed by the Personal Health Information and Protection Act. All requests related to PHI will be handled in accordance to that Act. For more information, please visit our Health Records page.

General Rules & Guidelines

  1. Any record of information, however recorded, whether in printed form, on film, by electronic means or otherwise which is in the custody or control of the hospital after January 1, 2007 is subject to FIPPA. Any person, including individuals or organizations, is able to make a request regardless of citizenship or place of residence.
  2. This right of access is limited by exclusions not covered by FIPPA and certain mandatory or discretionary exemptions.
  3. If the Freedom of Information (FOI) request involves more than one record, each record may be treated differently.

What Information is Accessible Under FIPPA?

Some information is made publically available on our website, and as such, it may not be necessary to use the provisions of FIPPA to access the information you are seeking. You may use the search function to find the information you are looking for.

Alternatively, you may review our inventory to make sure that SHHA keeps the type of information you are pursuing. Informal inquiries are welcomed.

How to Make an FOI Request

Should you choose to formally submit a request for information under FIPPA, there is a defined process to follow.

  • Complete the SHHA Freedom of Information Request Form.  Clearly identify the information you are seeking, and be sure to include your contact information.

  • Prepare a cheque or money order in the amount of $5, made payable to South Huron Hospital Association.
  • Mail your completed form and non-refundable fee to:

Alternatively, you may deliver your form and payment to SHHA by hand. Please Note: Due to legal requirements, emailed or faxed submissions are not accepted.
