Exeter Diabetes Program

The South Huron Hospital’s Exeter Diabetes Program is a proud member of the Huron Perth Diabetes Program, and provides inpatient and outpatient services to adult patients with pre-diabetes, gestational diabetes, Type 1 diabetes, or Type 2 diabetes.

The Exeter Diabetes Program Team is composed of Certified Diabetes Educators (CDE). These individuals are health professionals committed to excellence in diabetes management and specialize in helping you develop skills to manage your diabetes through education and coaching. CDEs must pass the Canadian Diabetes Educator Certification Board exam.


Referrals and Appointments

Your doctor may refer you to the Exeter Diabetes Program using this referral form. Alternatively, you may simply call our office to book an appointment.


Methods of care delivery include:

  • Face to face, in person
  • Telephone appointment
  • Video Call (virtual face to face)

Location, Hours & Contact Information

The Exeter Diabetes Program is located on the basement level of the South Huron Hospital Association. Services are available Monday to Friday, from 7:00 a.m. to 3:00 p.m.
