Four inpatient rehabilitation beds are available to provide a transitional care program with a range of rehabilitation services. Patients admitted to the rehabilitation program are often recovering from a stroke or an orthopedic condition (e.g. following a fracture), or have medically complex condition. The rehabilitation program at SHHA requires that clients can actively participate in and tolerate 30-60 minutes of therapy per day.
Patients are assessed with respect to the level and type of rehabilitation services they require. The rehabilitation services team includes:
The team works closely with the patient, their family and other care providers to ensure the patient recovery is optimal, care needs are addressed and discharge planning is effective.
The four complex continuing care beds are provided for patients who have long-term illnesses or disabilities typically requiring skilled, technology-based care not available at home or in long-term care facilities. The complex continuing care program is most suitable for patients with medically stable but chronic conditions. Patients who are discharged from complex continuing care may be able to return home, or to an assisted living arrangement in the community while recovery continues, or to a retirement home / long term care facility.

The team provides medical management, skilled nursing care and a range of interdisciplinary diagnostic and therapeutic services and technologies designed to meet the individual needs of patients. The team works closely with the patient, their family and other care providers to ensure that patient recovery is optimal, care needs are addressed, and discharge planning is effective.
