Quality Improvement Plan (QIP)

Quality improvement is an ongoing priority that helps us continually find new and better ways of doing things so that we can enhance care for patients, increase satisfaction and achieve even better clinical outcomes.

Our Quality Improvement Plan, or QIP, is one tool that we are using to help us document and review our current performance in a variety of areas. With this plan, we will be able to very clearly see our targeted areas for improvement and chart our progress.

How to Read the Quality Improvement Plan

Our Quality Improvement Plan (QIP) is made up of two parts:

1.      Part A:  The 2011-12 Quality Improvement Plan (Short Form) provides a brief overview of our quality improvement plan, highlighting and listing our hospital’s top priorities for the year.  To view this document  - click here.

2.      Part B:  This spreadsheet includes our improvement targets and initiatives and identifies a core set of indicators that all similar hospitals across the province are working on.  To view this spreadsheet - click here.

The QIP is only one of the ways we are working to improve our patients’ experiences. Not all indicators apply to all hospitals. In these cases, you will see “N/A”. This simply means that we are focusing on other important indicators.

Please feel free to contact us with any questions you may have or visit www.ontario.ca/excellentcare for more information about the Excellent Care for All Act and its requirements, including the Quality Improvement Plan.

QUALITY PLAN UPDATE - June, 2011

Improving hand hygiene compliance is part of the SHHA Quality Improvement Plan. A two-stage hand hygiene blitz, using Glo Germ, was planned to assist SHHA in reaching this goal. Glo Germ was used as a teaching aid and provided visual reinforcement on how to improve hand hygiene; therefore, reducing the spread of germs to other people.
